This survey seeks feedback from supported people, families, carers on how Angus Health and Social Care Partnership commission home and community-based care and support services under Self-directed Support Option 3 (SDS Option 3).

Your feedback will help us review and improve these services. We would be grateful if you could share your experiences of the care and support package you or your family member or cared for person receive under SDS Option 3.

SDS Option 3 allows you to ask Angus Health and Social Care Partnership to organise care and support for you or your cared for person. This includes arranging the service and making payments directly to the provider on your behalf.
